Our doctors and nurse provide tailored treatment including preparation of chronic disease management plans to help manage your chronic conditions (including but not limited to: Diabetes, Cardiovascular Disease, Asthma, COPD, musculoskeletal conditions).  A chronic disease management plan also provides access to 5 Medicare subsidised allied health visits each year to assist with optimising treatment and meeting goals.

We regularly offer review and take into account the following management factors:

• Medication management

• Blood test monitoring e.g. HbA1c, cholesterol, fasting blood glucose

• Medical imaging monitoring e.g. Bone density

• Cardiovascular assessments e.g. Heart health checks, ECGs, coronary calcium scores.

• EPC referral to allied health – e.g. Dietician, Physiotherapy, Podiatry, Chiropractor, Speech Therapy
